All participants in Northview athletics must have a valid physical form and insurance waiver on file with the Athletic Department.  All physicals taken after April 1st are valid for the preceding school year.  No physical form other than the Northview physical form will be accepted.  These forms are YOUR RESPONSIBILITY and will not be collected by the Touchdown Club.
